Adobe Reader (a free download) is necessary to view forms in “.pdf” format and some forms may be completed “on screen” using Adobe Reader. Use your “tab” key to move between fields on the form, print, and fax completed form to (817) 579-0213.
Forms in “.exe” format are OmniForm Mailable Forms and may be downloaded, completed on screen and returned via email to: general@aopoffice.com
Certain firewall and/or virus protection software may prevent you from downloading .exe files. Check with your system administrator.
Both .pdf and .exe forms may be downloaded and stored locally on your computer for future use. In Internet Explorer right click on the link and select "Save Target As", then specify the location on your computer where you want the forms to reside. You will then be able to utilize the forms without downloading them again from our site.
